Do you accept dental insurance?
We accept a variety of PPO plans. Contact our team with your plan information so we can check participation and benefits. We do not accept Medicaid or DMO plans.
What should I bring to my first visit?
Bring a photo ID, insurance information if applicable, a current medication list, and any relevant dental records or recent X-rays.
Can I book online?
Yes. The Book online button takes you to our appointment scheduler. For urgent concerns or help finding a time, call the office.
How do I find the office?
We are at 260 W Sunrise Hwy, Suite 201, Valley Stream. The office is on the second floor. Find us on Google Maps.
